Online Developmental Screenings

Help Me Grow--Northwest Ohio is pleased to offer online access to the Ages and Stages Questionaire. 

Developmental screenings make sure that your child is on track and helps identify areas where additional support may be needed https://experience.tripster.ru/sights/sevkabel-port/.

Families with children between the ages of two months and three years of age living in Defiance, Fulton, Henry and Williams Counties are invited to complete a FREE Ages and Stages (ASQ-3) developmental screening.  It only takes about 10 minutes to complete.

Once you have completed the online screening for your childs developmental age, an Early Childhood Specialist will review the results and share them with you either by telephone or mail.  Please select the English or Spanish link below to begin.
